To complete the application process:  
1. Collect documents required for processing the Mazur Fund Application:
– Dane County Residency proof (lease, current utility bill)
– 2 most recent bank statements
– 2 most recent pay stubs for all earners in household or verification of your sources of income (such as social security award letter, unemployment statements, etc.)
– 2 estimates (for car repairs only)    
2. Email them to apply@jssmadison.org or fax them to 608-819-6550.
3. Caren will not contact you until she has received your documents.
4. Once your documents are received, Caren will contact you and schedule an intake appointment to complete your financial assistance application.

Please also note that all JSS visits are by appointment only and walk-ins will not be accepted.
6. It is important to understand that this process takes time – please be patient and know that there is a limitation on funding and that funding is NOT guaranteed. Grants are for less than $1,000

Below are some additional resources for financial assistance in Dane County. If you have questions or other needs, please call 211. 

  1. WayForward Resources (food pantry and housing assistance) (608-284-0202) serve all of the Middleton Cross Plains School District and part of the west side of Madison (west of Midvale Blvd, and inside the beltline loop). Senior Rides Program: Volunteer drivers provide rides to a local destination, such as a medical appointment or grocery store. In order to be eligible, you must live within WayForward’s service area and be 60 or older. 
  1. Joining Forces for Families (JFF): Michael Bruce for Questions at 608-335-7817. Families with limited resources that have not received an Emergency Assistance grant already in the last 12 months can qualify for Emergency Assistance if: There is at least one minor dependent in the home. They have a 5-day eviction notice for non-payment of rent, or the household is currently homeless and has found new housing to move into. The group has less than $2,500 in assets. The household has less than 115% FPL of monthly gross income (before taxes are taken out). 
  1. Emergency Assistance (608-242-7458) provides a payment and a connection to resources for families facing a setback due to a housing emergency, including impending homelessness and domestic violence, natural disaster, fire, or an energy crisis. You can apply for EA online using the ACCESS website: https://dcf.wisconsin.gov/ea.   1. Individuals and families seeking eviction/homelessness prevention funds should call the Tenant Resource Center at (608) 257-0006, option 7.  Each household will be assessed and placed on a coordinated priority list, then will be contacted by the program for which the household is eligible. 
  1. Freedom Inc. (608-716-7324) advocates who help victims and survivors of domestic violence, intimate partner violence, and/or sexual assault get the help they need. These services are confidential and free. Who we help: Low- to no-income Black and Southeast Asian women, girls, and LGBTQ+ people in the Madison area. Services include:  

– Finding housing assistance
– Applying for federal or state benefits 
– Navigating the legal justice system 
– School advocacy 
– Fleeing domestic violence 
– Immigration and citizenship attainment
– Case Management 
– Navigating health system 
– Sexual assault advocacy
